Premature Ejaculation Definition and Diagnosis

Premature ejaculation is very common and will affect most men at some time during their life. It is more commonly occuring among younger men, as sexual inexperience is regarded as one of the contributing factors of premature discharge. Young adolescents also have more sensitive penis heads which they grow out of as they develop into men. Most men grow out of it, and learn to delay the ejaculation longer with each successive sexual partner, but others do not grow out of it, and become stuck as lifelong premature ejaculators.

A diagnosis of premature ejaculation is not very precise at confirmation of having the symptom, as there can be much variability in the exact length of time men last during different sexual encounters. Some men may be able to penetrate the vagina for a satisfactory length of time most of the time, yet occasionally experience a premature discharge. It is only when the problem is consistent, however, then there is more support to diagnosis the symptom as problematic.

The definition of premature ejaculation is the symptom characterized by a condition where the man ejaculates too fast in order to engage in any meaningful sex. Men naturally want to last longer, and may feel abnormal if they discharge too soon. They may see themselves as having premature emanation, but they might be average. A length of 3-5 minutes is very normal. On the other hand, where the problem is drastic, and is always less than 1 minute or even 30 seconds can be considered more problematic.

In order to extract a standardized time for defining premature ejaculation, Waldinger undertook a study by recordeding the median value of times to climax out of 491 men, with average 8 sessions of data from each man, totalling about 4000 in total, the median (halfway point) time was found to be 5.4 minutes. This is a good starting point by which to define an average length of penetration, yet there is still much variablity in the lengths of time from the sample as its skewness suggests a larger proportion of men will last less than this, and so the mean (average value) will be even less than the median. Premature Ejaculation Causes

The causes of premature ejaculation is unknown but believed to be a multitude of multiple factors both psychological and physiological, yet is thought more psychological, especially caused by a condition known as performance anxiety. This symptom can be self-perpetuating, because the overall lack of confidence in himself causes it to happen.

Other factors to consider, such as age and sexual experience of the man. The level of arousal or excitement at the time, with a higher level of excitement leading to more rapid ejaculation, this may occur with new partner or in new setting. If the frequency between ejaculations becomes longer, for example, after a period of ejaculation abstinence, may cause faster ejaculation the next time. Premature Ejaculation Treatment

The treatment for premature ejaculation involves learning control with a partner. Going for gradual improvement rather than total prevention and will require practise over several weeks or months. A popular exercise technique called the Stop & Start method involves stimulating yourself to just before the point of no return, at which point you stop stimulating yourself and prevent ejaculation. Do these exercises regularly with the aim of increasing the length of time it takes before reaching the point of no return.

As premature ejaculation is thought can be caused by low levels of serotonin in the brain, a certain mood enhancing brain chemical, is believed a contributing factor in premature ejaculation which was supported in animal trials. This theory was further supported by the evidence that administration of SSRIs (selective serotonin reuptake inhibitors), for example anti-depressants like paroxetine or dapoxetine, which alter levels of serotonin, dramatically improved the ability to last longer, with a large extension in average retention time upto 20 fold the original average time to emission.

A drug with the brand name Priligy, contains dapoxetine and is available to buy legally in some EU countries, otherwise in order to get a prescription for an SSRI from your doctor, you will need to be diagnosed with a condition like depression, mania, or bipolar disorder. It is unlikely you can get a prescription of an SSRI just to treat premature ejaculation. Yet you may be able to buy Priligy from some countries and obtain it without prescription. Other contenders in this space may mimic the effects of SSRI and be marketed to treat Premature Ejaculation, for example Dapovar is obviously a take on the name dapoxetine.

The use of SSRIs, however, are not without side effects, and potential side effects can be both physiological and also psychological. Using SSRIs alters your brain chemicals unnaturally, and withdrawal from using them can itself cause depression, the exact same thing it is trying to treat. In addition to the side effects, using SSRIs for premature ejaculation will cost a lot.

Another option is using anaesthetic sprays or creams to dull the sensations of the penis and reduce sensitivity. The problem with this is that the anaesthetic can also be passed on to the partner dulling the sensations for both partners. This will reduce overall pleasure by dulling sensations and may lead to less pleasurable sex.
